Summary:

The Case Management Assistant provides assistance to Village Health Providers in direct patient care and in coordination and management of administrative duties. Responsibilities include, but are not limited to: reception/ clerical tasks, database management, patient screening and rooming, follow-up tracking of patients with chronic diseases, assisting patients with activities of daily… living, obtaining vital signs, collecting specimens, performing point-of-care testing, preparing and cleaning patient care areas and equipment, and monitoring and ordering supplies.

Essential Job Functions:
• Assist Village Health Providers during remote village visits to room patients, record vitals, and assist as needed.
• Assist in reviewing and tracking all incoming requests for referral and consultation.
• Track requested testing is completed prior to patient appointment so care is not delayed.
• Assist as needed in arranging patient travel and send medical necessity letters as appropriate
• Arrange provider travel to villages as needed.
• Assist in maintaining database showing when village patients require follow-up so they can be scheduled for appointments during the appropriate provider village visit. and track when village patients need mammography, pap testing, colonoscopy, etc.
• Maintain provider travel calendar for village visits and assist with travel arrangements as needed.

Additional Functions:

May require travel to remote village to aid during provider visit by performing patient intake and vitals, if needed. Assist coworkers as needed. Other duties or special projects as assigned.

Licensure/Certification/Registration:
• CNA/EMT preferred
• Basic Life Support (BLS) certification required within 30 days of hire.

Education:
• High School Diploma or equivalent degree (GED), required

Experience:
• Six (6) months to one (1) year administrative assistant experience in a healthcare setting preferred.
• Clinic experience or past experience as a CHAP preferred.

Degree of Supervision Required:

Under supervision of Senior Director Medical Staff Services and the clinical supervision and direction of Village Health Provider, Village Health Case Manager, or designee.
